New Patients
Your initial appointment will consist of a clinical and radiographic examination along with a review of your treatment options if treatment is indicated. Additionally, a general cleaning will be completed. Sometimes, treatment can be done or started the same day as the exam.
Please assist us by providing the following information at the time of your exam:

Any x-rays taken by a previous dentist, if applicable.

A list of medications you are presently taking.

If you have dental insurance, please bring the necessary completed forms. This will save time and allow us to help you process your claims along with providing you a more accurate estimate.
MPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian.
Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and artificial joints, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
X-rays:
If you have recent dental x-rays you can contact your previous dentist's office to have them sent to our office. If your last set of x-rays were taken more than one year ago we can take a new set to complete your exam.
